what is kap?

KAP is an innovative approach that combines the dissociative-anesthetic medicine, ketamine, with the power of relational, integrative psychotherapy. It’s effective in treating a range of mental health conditions, including depression, anxiety, trauma/PTSD, OCD, chronic pain (e.g. fibromyalgia, migraines), and problematic substance mis-use, or for those who feel stuck, seeking insights, change, purpose, or psycho-spiritual exploration. KAP is notably effective for treatment-resistant conditions.

how does KAP work?

Ketamine induces an expanded state of consciousness that is highly effective for therapeutic re-processing and growth. It’s dissociative and anti-anxiety properties provide a unique space for clients to explore emotions, thoughts, and memories, without emotional constraints or defense responses of ordinary mind (e.g. fear, dread, dissociation). In this space, clients have greater access to their internal strengths, wisdom, and healing capacity, as well as positive emotional states (e.g. joy, creativity, love, empowerment, vitality), creating cognitive and emotional flexibility and expansion.

The kap session

KAP sessions are 3 hours, allowing for essential recovery and integration time. Emphasis is on providing psychotherapeutic support and processing material that arises during sessions. Dosing sessions can range from low to high, eliciting various, altered levels of consciousness, each with their own signature and therapeutic benefit. Doses are tailored to each client and their unique goals. The overall treatment process includes: psychological & medical intakes, preparation sessions, dosing sessions, and integration sessions.

  • Ketamine is a dissociative anesthetic. It has an extremely safe track record and has been used in surgical settings and pain management since its approval by FDA in 1970. It is now increasingly applied clinically as an off-label treatment for various chronic treatment-resistant mental health conditions.

    Ketamine works on the brain's neurotransmitter systems, particularly glutamate, which plays a crucial role in mood regulation, cognition, and emotional processing. It also promotes synaptogenesis and enhances neuroplasticity. Meaning, it helps relieve symptoms, offers new ways of thinking (especially about one’s life and big picture of existence), and helps grow new brain cells and connections.

  • Ketamine has various routes of administration. With KAP, it is either taken sublingually (SL) via a rapid dissolving lozenge, or intramuscularly (IM) via injection.

    For our KAP sessions, we will mainly be using rapid dissolving lozenges (SL). IM sessions are offered once a month, under the administration and supervision of a medical professional.

  • The number of KAP sessions is individual, depending on overall treatment goals and responsiveness to treatment.

    KAP has a cumulative effect, which means that it doesn’t provide significant benefits after a single treatment session. Our first ketamine session helps us determine the most appropriate dose for you. I recommend having a minimum of 3 KAP sessions, with clients typically benefiting from 3-6 sessions.

    Some clients seek ongoing “booster sessions,” intermittently over months or years.

    Before we begin treatment with KAP, I offer 2-3 preparation sessions so you feel comfortable, clear, and grounded going into the medicine experience. Following each KAP session, we will meet for an integration session. There is no specific schedule between sessions; everyone’s needs are different. In general, I recommend you wait at least a week between sessions.

  • On the day of your ketamine treatment, please follow the directions given at intake about food and medication.

    Each KAP session is 3 hours, including a 15-minute check-in before we begin to clarify intentions for your journey. We will optimize set and setting through the use of invocations, meditative practices, and/or breathing exercises. These contemplative practices help foster an open, relaxed mindset, reduce anxiety, and ease the transition into non-ordinary states of consciousness.

    Once the ketamine is administered, you will be encouraged to use an eye mask to facilitate the experience and maintain an internal focus. You will spend much of the session focusing on your internal experience. However, I will be by your side, checking in with you periodically throughout your experience. The ketamine journey itself typically lasts between 30-50 minutes.

    As the effects of the medicine subside, we will spend time discussing and integrating your experience. I will take detailed notes on all that you share to support recall, insight, and integration.

    If the medicine effects last longer than the allocated time, you will be able to recover in the waiting area until it is safe for you to leave with your arranged driver.

  • Ketamine effects are dependent on dosage, which is based on overall treatment goals and plan.

    Low dose sessions provide an empathogenic effect. It may result in increased awareness of the body, feelings of comfort and relaxation, reduction of ego defenses, increased empathy, compassion, and warmth. This is considered a sub-psychedelic dose and best supports talk therapy, as there is more openness and relaxation of defenses.

    Moderate dose sessions often involve an out of body experience (OBE), where you feel separated from your body. This results in significantly diminished ego defenses but with continued awareness of the Self. OBEs may also include mythological/archetypal imagery, visiting non-physical realities, possible encounters with spiritual entities and non-terrestrial beings, vivid dreams of past and future lives and incarnations, and re-experiencing aspects of the birth process.

    High dose sessions may involve full dissociation from body and various transpersonal and mystical experiences, including a near death experience and/or ego-dissolving experience.

    In the doses that I use, you will most often experience empathogenic and/or OBE at low to moderate doses.

  • KAP is not an appropriate treatment for everyone, especially for people with certain medical and psychological conditions.

    Psychiatric contraindication for KAP, include: psychosis, schizophrenia, or active mania or hypomania associated with bi-polar disorder.

    Medical contraindications for KAP, include: allergy to ketamine, cardiovascular disease or arrhythmia, uncontrolled hypertension, uncontrolled hyperthyroidism, or pregnancy & nursing.

    Further screening is necessary to determine whether or not KAP is a safe course of treatment for you.

Questions

  • KAP is a catalyst for change, not a magic bullet for healing. While many clients experience profound shifts, it is typical to continue therapy, and to expect growth to be incremental over time.

    Improvement generally occurs with more than one administration and is most robust when part of an overall treatment program.

    Potential benefits may include: improved mood, reduced anxiety, improved resilience, reduced hyper-vigilance, trauma resolution, increased engagement with life and relationships, increased self-compassion and mindfulness, decreased compulsive and addictive behaviors, and a wider-angle perspective on your lived experience and identity.

  • Ketamine has an extensive record of safety and has been used at much higher doses for surgical anesthesia, without respiratory depression. As with any medication, there are also some potential side effects and risks.

    Although not exhaustive, most common side effects include: nausea, vomiting, lightheadedness, brief-transient increase in blood pressure, changes in visual, tactile, and auditory processing, feeling suspended in space or floating, mental confusion, excitability, and diminished awareness of physical functions such as respiration.

    Due to risk of nausea and vomiting, patients will be advised to refrain from eating at least 4 hours prior to a session.

    Due to possible blurred and altered vision, as well as impaired balance and coordination, you will be advised to remain lying down and keep your provided eye mask on until the main effects have worn off.

    Driving an automobile or engaging in hazardous activities should not be undertaken on the day of the dosing session. You will be assessed for safety prior to leaving the office.

    Some people report that the transpersonal experiences are bizarre or frightening, while many describe them as joyful or fascinating. I believe that frightening or overwhelming experiences are part of the healing process, and often pivotal in the resolution of the suffering that clients were seeking treatment for in the first place. I’m here to help you navigate, process and alchemize these experiences.

  • Ketamine is a controlled substance (Schedule III). It does have some abuse potential, mainly in recreational settings. However, in clinical practice, given the limited access, psychoeducation of responsible use, and close monitoring, problems with the abuse and diversion of ketamine are rarely seen.

  • In medicalized models of ketamine (IV clinics and Spravato), the medicine is seen as the primary agent of healing, meaning your healing is dependent on taking a drug. In these settings, the altered state is also minimized and even seen as an undesirable side effect, with the biological impact of the drug being credited for positive change. To me, this is disempowering, placing clients in a passive role in their own life and healing. It also goes against current psychedelic research, which is finding that the altered state, particularly when it reaches a level often described as mystical, is the best predictor of positive change.

    In our work together, you are seen as the primary agent of healing, looking to your internal resources to catalyze healing and transformation. While the medicine is an important aspect of this work, it’s actually the therapeutic aspect that’s the vehicle for transformation.

    Looking at other ketamine treatment facilities, I have found practices that feel unsupportive, and, to me, even unethical. These include: having sessions as short as 40-min, using dosages with minimal therapeutic benefits, no mental health professionals on site, clients being left to journey alone, and minimal-to-no preparation or integration. I have also seen many practice settings feel unecessairly cold and clinical.

    In contrast, at Telos Collective, the aim is to create a warm, comfortable environment, with intention given to each part of the process. This includes: myself supporting you throughout all of your KAP journeys, careful selection of music designed to enhance the therapeutic journey, the use of eyeshades during sessions to deepen the experience, 3 hour-long session times to ensure adequate recovery and integration, post-session refreshments, and ongoing integrative psychotherapy.

  • Currently, insurance plans do not cover KAP. I’m an out of network provider, meaning I don’t take insurance, for intake, preparation, or integration sessions.

Preparation

While psychedelics hold potential benefits, there are risks associated with them. Adequate preparation and harm reduction are essential in supporting safe, meaningful exploration. Other offerings include: clarifying intentions, creating safe set and setting, and dieta guidance.

Integration

Integration helps you gain clarity, perspective, and wisdom from your medicine experiences. Integrating material from your journey shortly after your experience allows for deeper, longer-lasting change before the rubber band of your mind returns to its dominant state.

Ongoing support

Like therapy, work with psychedelics can often be slow and non-linear. Each encounter is like peeling back another layer of the proverbial onion, necessitating ongoing exploration, reflection and integration. Offering continued support through the ongoing arc of your psychedelic journey.
